Conditions Prior to Cccurrence:
The reactor was shutdown; caintenance outage in progress.
D:ceription of Occurrence:
As a result of perforr.ing a leen1 1-nk rate teet, it was discovered that the inboard cain occata line cceple isolation valve dirl not seal tightly.
The outboard isolation val"e was closed.
Designation of Apparent Cause of Occurrence:

Invcutination in progress.
Analycis of Occurrenco:
The other isolation valve in thia line tras verified to seal properly and vns left in tha cloced pocicion.
For this reason, the occurrence,hsa minical safety inplicaticas.
Correctiv6 Action:
Valve vill be repaired as required.
Failure Data:
Previcus failuren of this type vero reported an Abnornst Occurrence Report Numbers 50-278-74-15 and 50-278-75-1.
