P uch Bottom Atomic Power Station R.D. 1, Delta, Pennsylvan'.a 17314 Identification of Occurrencer The RER IICI infection valve HO-2-10-154B motor failure.
Conditions Prior to Occurrence:
Reactor operating at approximately 70% power.
l Description of Occurrence:
l During curveillance testing the valve failed to open. Investigation l
revealed that the motor had failed electrically.
Dcsignation of Apparent Cause of Occurrence:
i Possible rotor shaft slippage resulting in cechanical damage which then caused electrical failure.
Analysis of Occurrence:
Failure of this valve in the closed position resulted in an inoperable LFCI system. Surveillance testing was in=ediately begun on the diesel i
generators and core spray sub-cyste=a as required by the Technical I
Specifications. Prior to the completion of this testing, the valve was epened canually and left open. Because the valve in norcally in the crea positiert and in open during IJCI injection it was determined that the I.FCI sycte= was then operable. This occurrence, therefore..has minimum safety significance.
Corrective Action:
The valve motor has been reened and inspected. The cause of the damage will be deter =ined and the raotor vill be repaired or replaced. As indicated above, the valve has been manually opened and left in that position, thus rectoring the LPCI system to full operability.
