Conditions Prior to Occurrence:

Unit 2 start @g. up -epproxinstely 20% power.

uescription of Occurrences Darins a r'outine plant startup inspection of plant safety instrumentation, one of the reactor low water Icvol scram /roactor high water level HPCI turbine trip instrucents. (CIS.-2-2-3-101D)uas found to not have been properly returned to service following naintenhnce on its instrutaent test valve block.

Designatio' n of Apparent Cause of Occurrence:

The testing specified to be conpleted following naintenance was incoc:pleccia in that verification of correct valve lineup was not required.

Analysis of Occurrence:

The switch was found in the equalized condition which drives it full upscale and gives one of the two needed reactor high water level turbine trips for the HPCI.

The reactor Low Water Level Scran switches are arranged in a one-of-two twice logic chus, no one switch being disabled can render this scran inoperable.

Because the other three scran switches and the other reactor high water 1cvel

HPCI turbine trip avitch were operabic, the safety implication't are mininal.

Corrective Action:

This instrunent vaa tenediately backfilled, vented and returned to rervice.

Verbal instructions have been issued to remind personnel to be more careful') in specifying verification testing following naintenance.
