Description of Occurrence:

During routine instrument surveillance testing, PS-2-2-3-55C (Reactor High Pressure Scram switch) was fcund to be valved out of ser-vice.

Immediate checking of all other surveillance tested instruments revealed that another switch (PS-2-5-12A, Drywell liigh Pressure Scram) was valved out of service.

. Designation of Apparent Cause of Occurrence:

The cause for these occurrences is technician error in re-turning thesc. instruments to service subsequent to performing surveillance tests.

Analysis of Occurrence:

Each of these switches is part of a separate one-out-of-two-twice scram logic, and the defeating of any one switch in either logic would not have defeated the safety system logic. Therefore, the se occurrences have minimal safety implications.

Corrective Action:

All other surveillance tested instruments were checked for proper valve lineup immediately. The importance of strict adherence to procedure, and the verification of proper. valve lineup following each test has been impressed on each of the instrument technicians associated with this occurrence.

Additionally, an independent verification of post testing valve lineup on certain instrumentation systems has been added to the procedure.
