During mutino instrument surveillance testing, dPIS-2-2-138A was found to trip at 0 9 paid. The Technical Specification limit is 2 paid 11.

h* nation of Aeparent Cause of Occurrence:

Setpoint shift.

Analysis of Occurrence:

This minor setpoint shift has minimal safeyf implications since it is part of a one-of-two-twice logic and is also redundant in the other LPCI systet.

• Corrective Action: _

'Ihe cem follower was found to be defective. It was replaced, ,

and properly aligned. The switch was recalibrated and check for proper calibration per the applicable surveillance test.

Failure Data:

This is the first failure of this type on switches in this serviCo.
