Abnormal Occurrence The folicwing occurrence was reported to Mr. Walt Baunack, A.E.C. Region 1 Regulatory Operations Office on November 25, 1974.

Written notification was made to Mr. James P. O'Reilly, Region i Regulatory Operations Office on November 25, 1974. In accordance with Section 6.7.2.A of the Technical Specifications, Appendix A of DPR-56 for Unit 3 Peach Bottom Atomic Power Station, the following re-port is being submitted to the Directorate of Licensing as an Abnormal Occurrence.

Occurrence Date: November 25, 1974 Facility: Peach Bottom Atomic Power Station R.D. 1, Delta, Pennsylvania 17314 Identification of Occurrence:

  • HPCI isolation following automatic initiation.-

Conditions Prior . to Occurrence:

Unit 3 reactor-isolated and scrammed as part nf a startup test program.

Description of Occurrence:

During the review of data taken during the 100% reactor isolation test performed on 11/19/74, it was noted that the HPCI had isolated after receiving an initiation signal during the. transient. The unit'was 3 n"

Designation of Apparent Cause of Occurrence:

A review of the test data and interviews with the operators present during the transient has not provided on absolute cause.

Analysis of Occurrence:

During this transient, the reactor water level momentarily dropped to the initiation icvel for HPCI and RCIC. Reactor watcr level was immediately recovered by way of the reactor feedpump. HPCI opera-tion was not required during this transient. All~other engineered safeguard systems operated as expected. This occurrence has minimal safety implications.

Corrective Action:

In an attempt to further investigate and determine the exact cause of this occurrence, a HPCI quick start with a condensate storage tank to condensate storage flow path was performed on 11/27/74. During this test, MPCI stop valve movement, control valve movement, high steam line flow /4p indication, speed, flow and discharge head were monitored.

All parameters moni tored were satisf actory and no HPCI system isolation occurred. A vessel injection test will be repeated prior to completion of a 1004 testing program to further ensure HPCI operability.
