Drywell equipment drain sump isolation valve A0-3-20-74

failed to close on a PCIS isolation.
Conditions Prior to Occurrence:
Unit 3 operating at 3182 MWT, 96.6% power.
Startup Test 27, Turbine Trip had just been performed causing reactor scram and group 2 and 3 PCIS isolation.
Description of Occurrence:

During the PCIS isolation, A0-3-20-94 failed to isolate.

The valve also failed to isolate on a manual closure signal.

Mr. A. Giambusso December 31, 1974 Page 2 Designation of Apparent Cause of Occurrence:
The solenoid valve which de-energizes to bleed the air off the diaphram stuck in the energized position preventing valve closure.
Analysis of Occurrence:
The other isolation valve in this line, A0-3-20-95, did close to isolate the line. Therefore, there is minimal safety im-pli ca tion.
Corrective Action:
The solenoid valve was rebuilt and satisfactory operation was veri fied wi thin 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> of the occurrence.
Failure Data:
Similar occurrences were reported as Abnormal Occurrence Report Numbers A0-50-277-74-7 and A0-50-278-74-26.
