Conditions Prior to Occurrence:

l Unit 2 at 50% power.

Description of Occurrence:

On June 7,1975 the Unit 2

'B' containment spray inner valve (MO-318) was identified as electrically inoperable during the per-formance of a routine surveillance test. The investigation completed on June 9,1975 showed that the bolts which attach the valve yoke to i

the valve body had failed. The valve was found in an open position.

Eailure appears to have been caused by a naifunction of the valve limi torque operator.

Investigation is continuing.

Analysis of Occurrence:

- This occurrence has minimal safety significance since all containment cooling sub-system loops were operable even with this valve out of service and open.

Following a LOCA, containment cooling is achieved by circulation of torus water through the RHR heat ex-changer and back to the torus. Heat is renoved via. the high pressure service water system using the RHR heat exchanger. The requi renent l

for the containment spray is an option availeble to the reactor oper-ator and must be initiated manually. Containment spray capabilit'y was available via the 'A8 loop containment spray valves ?nd by opening the b

outer 888 loop containment spray valves.

Since the failure of the 'Bt containment spray inner valve, the outer valve' in the same line has remained closed. This prevents any possible containment leakage.

Corrective Action:

i Investigation to dato revealed a failed motor rotor and sane malfunction of the limitorque valve operator or limit switch assembly.

Replacement parts are on order and investigation is continuing.

Until repairs can be made, the valve will be maintained open l

and the outer 888. loop containment spray valve shall remain closed (M0-268) and shall be logged in this position daily. The RHR contain-l ment cooling system is considered fully operable in this condition since both containment spray and suppression pool cooling flow paths can be initiated if requi red.
