'd 77-75-31/50-278-77 25 Mr. A. Giambusso May 8, 1975 Page 2 Designation of Apparent Cause of Occurrence:

A small particle of dirt or other foreign material is be-lieved to have. interfered with the field flashing logic relays.

The contacts were manually cycled and the diesel generator was restarted successfully.

Analysis of Occurrence:

The safety implications of this occurrence are minimal be-cause this' occurrence had no ef fect on the remaining diesels and one -

diesel is considered redundant.

In addition, the amount of tine that the diesel was inoperable was less than an hour.

The manual cycling of the ficid flashing relays was followed immediately by two satis-factory start s.

Corrective Action:

The manual cycling of the field flashing relays apparently removed the suspect particle of foreign material because the correct logic sequence was witnessed during subsequent starts. This diesel was started and synchroni zed for the next three days following this occurrence and operated properly on each occasion. The normal full load surveillance. test was performed satisfactorily on May 5, 1975 Failure Data:

No previous failures of this type.

Very truly yours, M.
