3A Core Spray prull Flav Test valve - MO-3-1L-26A failed to clos.
Conditions Prior tg occurrence:
Reactor power - 55.2%.
Description of Occurrence:
During the performance of surveillance testing, subsequent to valve
' maintenance. MO-3-1k-26A failed to close from the vide open position during operation of both core spray "A" loop pumps at rated flow.
During single and zero pump operation the valve closed satisfactorily.
Designation of Apparent Cause of Occurrence:
Under investigation.
Analysis of Occurrence:
Because the valve is only opened for a short period of time once a month during surveillance testing, the redundant systes was opereble, and because the operator conducting the test would have bees able, l
to stop the pu=ps, close the valves, and restart the pu=ps, there is mini =al safety significance.
Corrective Action:
Will be deter =ined subsequent to investigation.
