50-278-75-52 August 29,' 1975'-4 Pega 2 Designation of Apparent Cause of Occurrence:

Instrument setpoint dri f t.

Analysis of Occurrence:

The device is one of four arranged in a one-out..of-two twice logic (which ini tiates a Group III isolation on reactor zone ventilaa tion exhaust high radiation) such that the high trip setpoint would not have prevented the isolation from actuating at the correct point.

For this reason, and the fact that the device was still functional, the safety implication of this occurrence is minimal.

' Corrective Action:

The setpoint was adjus ted, the calibration checked, and the surveillance test was successfully rerun on the 3A monitor.

Several days were allowed to elapse, then the device was rechecked to deter-mine if any further setpoint drif t would be evident. The recheck in-dicated that no further drif t had occurred.

Failure Data:

No previous failures of this type.

Very truly yours,

)
