The dryvell floor drain inboard isolation valve (AO-3-20-82) failed to close 'Euring surveillance testing.
C or.di tions Prior to Occurrence:
Hot standby.
Description of Occurrence:
During the perfor ance of a surveillance test,-AO-3-20-82 failed to close when given an isolation signal.
Designation of Apparent Cause of Occurrence:
Under investigation.
Analysis of Occurrence:
The safety in;11 cations 'of this failure are minimal because the outboard isolation valve did cloce from the isolation signal.

The outboard isolation valve was left in the closed position.
