Setpoint shift on RCIC Steam Line Hi Plow Isolation Switch dP15-3-13-83 Conditions Prior to Occurrence:
Unit 3 shutdown.

During routine instrument surveillance testing, dPIS-3-13-83 was found to trip at 512 inches H20 The Technical Specification limit is < 450 inches H2O

w ignation of Apparent Cause of Occurrencet Binding between the can followe; roller skirt and the can; unevenness of the roller on the can follower.
j ilysis of Occ,urrence:
The safety implicati_ons are minimal since this switch was operable at less than the design limit of 300% flow (600 inches H2O differential), each switch acts independently to isolate the systers,, and the cc wanion switch was tested and found to trip within the Technical Specification limits.
crrective Action:
-Investigation tevealed two' things:
(1) there was some apparant binding between h
the can follower roller skirt and the can (2) unevenness in the roundness of the roller which causes setpoint shift.
The ceta follower was replaced; properly aligned; setpoint calibrated; and the switch che cked for proper calibration per tb applicable surveillance test.

Similar setpoint shif to reported as Abnormal Occurrences 50-277-74-41, 50-278-74-2 and 50-277-74-49.
