O'Reilly, Office of Inspection and Enforument, Region 1, United States Nuclear Regulatory Cmmission on February 6, 1975.
In accortlance with Section 6 7.2.A of the Technical Specifications, Appendix A of DPR-56 for Unit 3 Peach Bottcm Atmic Power Station, the following report is being submitted to the Directorate of Reactor Licensing as an Abnormal Occurrence:

Conditions Prior to Occurrence:
Unit 3 shutdown.

During routine instrument suiveillance testing, dPIS-3-13-83 at 512 inches H 0.
The Technical Specification limit was found to trig 2 2

Binding between the cam follower roller skirt and the cam; unevenness of the roller on the cam follower.
Analysis of Occurrence:
The safety imnlications are minimal since this switch was operable at less than the design limit of 300% flow (600 inches H O 2
differential), and the companion redundant switch was tested and found to trip within the Technical Specification limits.
Corrective Action:
Investigation revealed two things:
(1) there was some apparent binding between the com follower roller skirt and the cam (2) unevainess in the roundness of the roller which causes setpoint shift. The cam follower was replaced; properly aligned; setpoint calibrated; and the switch checked for proper calibration per the applicable surveillance test.
Failure Data:
Similar setpoint shifts reported as Abnormal Occurrences 50-277-7h-bl, 50-278-7h-21, and 50-277-7h-h9 Very truly yours, M.
