m 's Mr. A. Giambus> 50-278-75-19 April 1,0, 1975 Page 2 i Designation of Apparent Cause of Occurrence: Setpoint shift. i Analysi s of Occurrence: The contacts of this device are arranged in a one-out-of-two t twice trip logic. The setpoint of this device, not the trip function, was unsati sf actory. All other devices monitoring the parameters of condenser vacuum were immediately tested and found to be operational ] and correctly calibrated. Because of the above, this occurrence has minimal safety implications. Corrective Action: The out-of-tolerance pressure switch was immediately recali-brated to within the allowable limits of the Technical Specifications and calibration checked in accordance with the applicable surveillance test. The other three devices monitoring the same variable were tested and found to be satisfactory in both function and setpoint. Failure Data: Seven occurrences of this type have been previously reported A0-50-277-74-1, A0-50-277 19, A0-50-278-74-24, A0-277-74-35, as: A0-50-277-74-43, A0-50-277-75-1, and A0-50-277-75-19 Very truly yours, i j M.
