Pailure of LS-5222 to operate SV-5222 (RER loop "B" high point veut on lov accumstator level).
Conditions Prior to Occurrence:
Unit 3 cpe) ating at approx 1nately COI.
Descriptics of Occurrence:
Lair.u a routine instrur:ent surveillance test, the svitch function of LS-5222 which picks up SV-5222 to cpen the RER Loop "B" high point vent failed to cperate.
Th* alarm function of the switch did. operate properly.
Designation of Apparent Cause of Occurrence:
Eroken wire on LS-5222.
Analysis of Occurrence:
Since the FTA loop was full prior to the test, the low acct::mulator level alatT2 was proven functional, exd the operator vestid respend to the lov level alarn to ensure that the systata was t:.aintained full, the safety implications of this failure are niniral.
Corrective Action:
Investigation revealed that a vire within the LS-5222 switch case had been pinched and brs en.
The virin3 was repaireI end the lovel switch succescfully tested in cecordance with the applicable surve.111ance test.
