,17314 Identification of Occurrence:

Failure of LS-5222 to operate SV-5222 (RHR Loo'p "B" high point vent on low accumulator level).

Conditions Prior to' Occurrence:

Unit 3 operating at approximately 60%.

Description of Occurrence:

During a routine instrument surveillance test, the switch function of LS-5222, which operates SV-5222 to open the RHR Loop "B" 9 q' 'q V --

high point vent, failed. The alarm function of the switch did operate proper l y.

Broken wire on LS-5222.

Analysis of Occurrence:

Since the RHR loop was full prior to the test, the low accumulator level alarm was proven functional, and the operation would respond to the low level alarm to ensure that the system was maintained full, the safety implications of this failure are minimal.

Corrective Action:

I Investigation revealed that a wire within the LS-5222 switch case had been pinched and broken. The wiring was repaired and the level switch successfully tested in accordance with the applicable surveillance test.

Failure Data:

One previous, reported as 50-277-75-4.
