Description 'of Deficiency

. Durits preoperational testing it was' revealed - that the following

_ deficiencies were noted on the 125V DC bus 1E-PKB-M42:

, A.~

AC. power-supplies for the IE annunciators,' tag numbers 1J-RKB-UA-2D and 1J-RKB-UA-4E, were superimposing AC spikes on the1125V DC bus.

The AC superimposed spikes were causing erratic signals on the Diesel Generator "B" speed indicator-through 125V DC

' distribution panel IE-PKB-D22.

C.

Further~1nvestigatians revealed-the problem is common to all similar annunciators on Unit 1, 2, & 3.

It' has been determined that the 1E annunciator power supplies are-theiroot source of the AC spikes causing false Diesel Generator

. speed indications, while-the Diesel Generator was not in use.

The 1E annunciators were. supplied by Beta Products Incorporated.

II.

Analysis of Safety Implications The 125V DC vital bus. is provided to power emergency equipment during. a Design Basis Event (DBE).

Stray or superimposed AC signals of'a' magnitude that would impair or render instrumentation or equipment inoperable is unacceptable.

The AC spikes. generated from the 1E' annunciator power supplies are

being distributed to the diesel generator DC control circuits. This condition is producing a full scale false reading of approximately 700 RPM on the diesel generator REH meter,-while the diesel i-generator is not running. The full scale reading of the RPM-meter is above -the shutdown setpoint of the standby lube oil pump, which must remain operational to provide the fast starting requirements of the diesel-generator. In addition, the full scale reading of 'the F

RPM meter prevents the implementation of the diesel generator air starting sequence.

Based on the above, this condition is evaluated as Reportable under the ~ requirements of 10CER50.55(e) and 10CFR 21, since if this ccndition were to remain uncorrected, it would represent a significant safety condition.

One (1) annunciator from Unit 2 was returned to the Beta factory, based on the Beta field representative's recommendation, for investigation of the problems. - As a result of _the investigation, Beta will. install AC filter components to reduce and/or eliminate the AC spike interference to an acceptable level. The remaining annunciators for Units 1, 2, & 3 will be returned to Beta on a scheduled basis, to incorporate the required rework.

Project !?ocumentation will be updated to address the environmental and seismic qualification of the modified equipment.

B.

NCR SE-2309 and future NCR's will be dispositioned as " Rework."

C.

A copy of this report will be sent to Beta Products Incorporated requesting their review for reporting under 10CFR Part 21, including number and location of all components supplied.
