Designation of Apparent Cause of Occurrence:
3 The dP switch DIS-00014 which actuates the "B" f an was found stuck in the open position. This precluded activating the logic circuitry for the "B" fan start.
Analysis of Occurrence:
During the test on the reactor building ventilation isolation-signal the "A" SBGTS f an started automatically.
In addition, the "B" fan operated satisfactorily manually.
Safety implications are, therefore, minimal.
Corrective Action:
l e
Corrective maintenance was performed on actuator plate hinge and the auto start mode for the "B" fan tested satisfactorily several times.
l Failure Data:
A previous failure of this type was reported as A0-50-277-75-40.
Very truly yours, g
