Dear Sir:

To supplement previous discussions, Vernont Yankee Nuclear Power Corporation provides the following infornation relative to the December 27, 1972 transfomer failure at the Vermont Yankee Nuclear Power Station.

i.

Justify the new transfomer installation based on the total nomal con-nected steady state and starting loads, including engineered safeguard loads. Provide power and control schematic diagrams for the system.

On Decenber 27, 1972, the startup transfomer failed with no previous indication of trouble. The transfomer differential and sudden pressure rehying operated to isolate the transfomer. Since the plant was operating with the startup transfomer as the offsite power source, the plant experienced a loss of nomal power, resulting in a plant trip.

The startup transformer had to be returned to the manufacturer for repairs, so a temporary startup source was installed to allow plant operation. The startup transformer normally supplies the plant cuxi-11ary loads with two of its secondary windings and supplies the plant cooling tower load with its third secondary winding, as shown on the attached one line diagran, sheet 1.

Because of the limited avail-ability of replacement transformer capacity, one transformer was used to supply the plant auxiliary loads and the plant cooling tower loads were supplied with a separate arrangement of two transformers.

The startup transformer secondary windings were rated at 17.5 mVA for i

each winding supplying the plant auxiliary loads, and 12 mVA for the l

cooling tower loads.

'Ihe replacement transfomer has one secondary winding and is rated at 25 mVA at its full cooled rating.

Mr. Roger S. Boyd February 12, 1973 plant auxiliary load at full power is 20 megawatts, the cooling tower load is 9 negavatts, and the non-running safety load is 2.1 megawatts on each emergency bus.

Ch1culations indicated that the voltage dip caused by the replacement transfomer impedance would be approxinately 6.5 percent when the safety load is started in addition to the normal plant auxiliary ' load. Since this voltage dip is comparable to the calculated voltage dip of 5 2 percent caused by the impedance of the original startup transformer vinding, the capacity of the replacement trans-fomer was judged to be adequate and subsequent testing verified this judgment.

After the failure of the startup transfomer, the cooling tower loads were supplied by an installation of two transformers. The source for cooling tower loads was the tertiary winding of the plant autotransfomer which is rated at 62 5 nVA,13 8 kV. A 10.5 mVA transfomer converted this voltage to 3h.h kV.

A 15 nVA portable substation, located adjacent to startup transfomer, transfomed this voltage to h.16 kV which was connected to the cooling tower bus. A tanporary polo line across the plant yard was used to connect the tuo transfomers. This installation is shown on the attached one line diagram, sheet 2, and was used for the period of operation from January 10 to January 17 The plant was then shutdoun for fuel reconstitution.

In order to make the portable substation available for other energency use in the pouer system, the cooling towcr transformer arrangement will-be changed prior to the plant startup after fuel reconstitution.

The neu transfomer arrangement will consist of a 50 mVA,115/h3.8 kV trans-former uhich will be connected to the 115 kV line near the startup trans-fomer. The h3.8 kV output of this transformer will be converted to h.16 kV using a 7.5 nVA and a $ nVA transfomer connected in parallel.

This arrangement is sholm on the attached one line diagram, cheet 3 The original differential and sudden pressure relay protection for the startup transformer has been retained, but with the installation of the replacement startup transfomer it was necessary to me.ke minor changes in relaying and netcring circuitry. Since the replacement transfomer does not have a separate vinding for the cooling touer load, the transformer differential connections for that winding are not required. The watt-hour meter current transfomer connections on the 115 kV side of the transfomer were changed to accomodate the current transformer arrange-ncnt on the replacement transfomer. These changes shown schematically on the attached diagran, cheet 2.

The attached diagrams (cheets 300, 302, 306, 308, 328, and 1150) show the circuitry necessary to verify the independence of the relaying and controls on buses 1 and 3 No changes have been made in these circuits.

Shoot 300 - connections for the loss of nomal power relay, 27/STX.

Shcots 302 and 308 - DC control powcr to buses 1 and 3, respectively.

The attached diagrams (sheets 303, 305, 309, 311, 331, and 1163) show the corresponding circuits for buses 2 and h.

Sheet 317 shows the 27/STX and 27/ STY relay connections for the loss of nomal pouer cir-cuits for both trains. No changes have been made in these circuits.

2.

Provide a description and the results of the preoperational testing program perfomed for the new transfomer installation.

After the replacement transfomer was installed, preoperational testing was perfomed to assure the adequacy of the new installation.

To verify the capability of the transfomer to carry plant auxiliary load and in addition to start the full safety load, the plant was loaded with all najor plant loads and other loads required for 100 percent power operation to the extent possible. With this load of 16.25 megawatts on the transfomer; the RHR, Core Spray, HPCI, and RCIC initiation logics were simultaneously actuated. When these safety systems simultaneously started, the voltage dip was 18 percent at the h.16 kV suitchgear, an amount less than that experienced when starting the equipment on the diesel generator. The acceptance criteria for the test was net in that all required safety systems were satisfactori3y initiated.

To verify the integrity of the loss of nomal pouer circuitry after the new transformer installation, the relaying for loss of power from the offsite source (27/STX and 27/ STY) uas operated on each bus with the RHR and Coro Spray punp breakers in the test position. Upon application of the signal, the proper operation of the loss of nomal power relays, pump circuit breakers, and diesel generator circuit breakers on each emergency bus was verified.

3 Provide your assessment of the cause of failure of the transformer, including the possibility of any detrimental effect of the previous transfomer failure.

Testing perfomed at the plant site after the failure of the startup transforner indicated that the failure was due to an internal fault, either in the Y secondary uinding or between the Y and Z secondary uindings. This preliminary evaluation is based on megger tests.

Be-cause of the const;uction of the transfomer, it is impossible to pcr-form a visual inspection of the transformer internals until the trans-former is untanked at the manufacturcr's plant. It is anticipated that visual inspection will allow further assessment as to the cause of the failure of the transformer.

The transformer is expected to arrive at the nanufacturer's plant on approximately February 23, 1973 5

-h-February 12, 1973 When the unit auxiliary transformer failed on November 8,1972, the cooling touer winding of the startup transformer (Z winding) was carrying the cooling tower load. The two st,artup transformer windings which supply the plant auxiliary loads (X and Y vindings) were not in use, because the unit auxiliary transfomor was carrying the load.

Innodiately prior to the fault on the unit auxiliary transfomer, breaker 13 was manually closed; paralloling the X winding of the start-up transfomer with a unit auxiliary transfomer secondary winding through bus 1 and breaker 12.

5his connection was nade out of synchro-nism through an operator error. Within a feu cycles (breaker operating time) the closure of breaker 13 tripped breaker 12.

Almost immediately after this trancfer, the unit auxiliary transfomer failed.

For the duration of the transfer, the Z secondary winding of the startup trans-former was carrying the cooling tower load, the Y vinding was open circuited, and the X winding had inposed on it the out-of-phase voltage on bus 1 through breaker 13 It was apparently the Y winding that subsequently failed. There is a possibility that this failure could be a result of the out-of-phase condition on the X winding while the Z winding uas carrying load and the Y winding was open circuited. This possibility cannot bo fully evaluated until the visual inspection is completed.

h. What is the anticipated time period for this temporary power installation?

It is anticipated that this temporary power installation vill be in service until the startup transfomer is returned, installed, and tested. Present estinates of repair, delivery, and installation times indicato that the startup transformer may be reacbr for re-installation by approximately September 1,1973

5. What changes are planned in operating procedures as a result of the December 27, 1972, transfomer failure and transient and plant modifica-tions?

After the installation of the replacement startup transfomer and the cooling tower transfomer arrangement, Operating Procedure, OP h02, was written to provide instructions for operation of the temporary transfomer installations. The procedure providos details for nomal and abnornal operation of the transfomers and designates surveillance data to be recorded periodically.
