ABNORMAL OCCURRENCE REPORT
. Report No.--BFAO-73% W Report Date--December 7,1973 Occurrence Date--Hovember 30, 1973 Facility--Browns Ferry Nuclear Plant unit 1 Identification of Occurrence Unit 1 main steamline high-flow pressure switch malfunction.
Conditions Prior to Occurrence j
Reactor was at 25-percent thermal power during startup testing.
ll Description of Occurrence During routine daily instrument checks, main steamline high-flow pressure I
switch PdIS-1-25D was found not indicating correctly.
Analysis of Occurrence One switch from each of the four steamlines is connected in a series to establish one trip logic in a one out of two taken twice trip system in the primary containment isolation control system. The other 15 switches

were indicating correctly.
Corrective Action A replace =ent differential pressure switch, Earton Model 278, was installed, calibrated, functionally tested, and placed in. service. A bench test of the failed switch indicated that the low-pressure bellows was leakira.
Failure Data Barton Model 278; serial No. 278-2990.
