Dear Mr. O' Leary:
This letter is written to report an event at the Palisades Plant which was diccovered on May 14,197h and has been tentatively clannified cc an abnormal occurrence (Th-03). This was previously reported to the Directorate of Regulatory Operations, Region III, by telephene and telegram en May 15, 197h.
During their rcunde en the evening of Mey lb,197h, plant opero. toro discovered that a horizontal pipe restraint on tne suction line of lev-preccure safety injection pump F67 B was pulled loose from the concrete pil.lar to which it vac mounted. An investigation is being conducted into this catter; however, the investigation has not yet re-Yealed the purpose of the restraint or the cause of the bolted plate pulling out of the concrete.
The investigation is continuing and the results will be reported in the format prescribed by ReCulatory Guide 1.16 prior to returning the plant to cervice.
