4 ROCKETS 50-266 AND 50-10.1 DETAILED CONTROL ROOM DESIGN REVIEW IMPLEMENTATION UPDATE rOTNT BEACH NUCLEAR PLANT. UNITS 1 AND 2 Our letter to you dated December 21, 1990 (NRC-90-131) provided a schedule for implementation of the outstanding Detailed Control Room Design Review (DCRDR) upgrades at Point Beach Nuclear Plant.
This letter provides revisions in the implementation plan and schedule for correcting certain Human Engineering Discrepancies (HEDs).
These revisions involve DCRDR upgrades in the areas of power supplies for communications equipment, meter faces for electrical power supply indicators, the Control Roor ventilation contral panel, indicator crystal replacement, meter faces for turbine and rod control, hydrogen concentration indicators, rod control counter reset switches, procedura' changes, meter face range adjustments, Spray Additive Tank iniet flowmeters and meter face pointers.
POWER SUPPLIES FOR COMMUNICATIONF EOUIPMENT:
Human Engineering Discrepancies 152, 343, 344, 579, 598, 606, 781, and 782, all covered under modification request MR-87-216, will not be able to be completed by the scheduled installation date of December 31, 1991.
This modification request called for the installation of an uninterruptible power supply for the Gaitronics and radio equipment from an existing instrument bus.
- However, testing indicated that placing this additional load on an instrument bus could possibly cause voltage spikes.
These voltage spikes could result in spurious turbine runbacks and reactor trips.
In addition, the Gaitronics, the riant's public address system, is not classified as safety-related, and grounds or short circuits in that system could have adverse effects on the reliability of the associated vital instrument bus.
For these reasons, we have decided to power the Gaitronics and radio equipment through a planned non-safety-related station battery.

r Document Control Desk October 3, 1991 Page 2 The installation of this non-safety related battery is covered under modification request MR-90-150.
This installation is scheduled to be completed during the Unit 1 refueling outage in Spring 1993.
Once installed, this modification will allow additional flexibility for future plant modifications and will reduce the loading on the safety-related batteries.
After MR-90-150 is completed, we expect that six months will be required to install modification MR-87-216.
The revised MR-87-216 now calls for the installation of a new inverter and A.C.
distribution panel to power the Gaitronics and radio equipment.
This inverter vill be connected to the new non-safety-related battery.
HED's 152, 343, 344, 579, 598, 606, 781, and 782 are classified safety-significant HED's under the Point Beach Nuclear Plant DCRDR scheme.
We had planned on the resolution of all safety-significant HED's by the end of 1991.
However, these HED's are associated with MR-87-216, and they will not be closed out until this modification is completed.
In the interim, the plant communications systems currently available in the Control Room are sufficier:t for the following reasons:
1.
Gaitronics has an automatic transfer switch which selects power between a normal power source (36E) and an alternate t
power source (34E).
Emergency lighting panels 36E and 34E have the emergency diesel generators G-01 and G-02 as backup power sources, respectively.
2.
The radio repeaters have a manual transfer switch which selects power between two power sources, B33 and B43.
These power sources are backed up by G-01 and G-02, respectively, but are stripped on SI (Safety Injection).
3.
The King Radios are available for use.
These radios are hand-held portables and do not require any AC power source.
Operators normally carry these radios.
4.
Operations can use the sound powered phones which do not require any AC power.
5.
Operations can use the PBX dial telephone system which has its own battery backup.
There are telephonca at various locations in the plant where the operator may have to go, such as the auxiliary feedwater pump room.

s Document Control Desk October-3, 1991 Page 3 METER FACES FOR ELECTRICAL power SUPPLY INDICATORS:
The resolution of HED 10 calls for color banding the meter faces of the Diesel Generator frequency meters on panel CO2.
This color banding would be used to indicate the failed position of these meters at 60.5 Hertz.
It has been determined that this color banding is not necessary_for several reasons.
First, the operators know that the de-energized position for these meters is 60.5 Hertz.
If they were trying to adjust Diesel Generator frequency and the position indicated on the meter did not change, they would suspect possible meter failure.
Second, NUREG-0700 does not require meters in the Control Room to have the de-energized position indicated on the meter face.
For these reasons, no further action will be taken on this HED.
The resolution of HED 32 calls for the modification of various meter faces on Control Room panels IC04, 2C04, 1C20, and 2C20.
The meter faces on 2C20 will be modified in 1992, in conjunction with the Point Beach Nuclear Plant calibration program for Regulatory Guide 1.97 electrical meters.
This program is discussed in a letter dated July 5, 1991 (NRC-91-064).
This calibration program will start during the Unit i refueling outage in 1992.
The reason that these meter faces will be modified in 1992 is that circuit modifications will be performed which will install test switches for these meters.
These test switches will ensure the safety of personnel performing the required meter calibrations.
Addition-ally, the meter faces associated with the Reactor Coolant Pump ammeters will be replaced during the Unit i refueling outage in 1992.
All the remaining meter faces under this HED will be modified by the end of 1991.
The resolution of HED 43 calls for the modification of various meter faces on Control Room panels C01 and CO2.
The meter faces, with the exception of the battery voltmeters and the bus section 4 345KV voltmeter, will not be modified because these meters are not accident-related.
The benefits to be gained by reducing'the number
~
of graduations between numerals on the meter face from 14 to 9 to be consistent with NUREG-0700 guidelines do not warrant the extensive efforts that would be required to implement this modification.
The battery voltmeters on CO2 have been removed from l
CO2, and new battery voltmeters have been installed on panel 2C20.
l The bus section 4 345KV voltmeter will undergo the modification in 1991 to reduce the number of graduations between numerals.

-Document Control Desk October 3, 1991 Page-4 The resolution of HED 189 also requires changing the number of graduations from 14 to 9 on various_ meter faces on Control Room panels C01R, CO2R, 1C04R, and 1C04.
For the same reason discussed in the resolution of HED 43, this modification will not 'ce performed on the incoming, running, and gas turbine generator voltmeters on Control Room pancl CO2R.
Howevor, this modification will be performed on all the remaining meters by the end of 1991 as previously scheduled.
For further information on HED's 10, 32, 43, and 189, the revised Human Engineering Discrepancy records are attached.
CONTROL ROOM VENTILATION CONTROL PANEL:
The resolution of HED 339, covered under MR-88-114, calls for_the design and construction of the Control Room ventilation control panel.
This modification is currently scheduled for completion by December _ _15, 193t.
After completing the design phase, this panel was installed in the Point Beach-Nuclear Plant simulator.
After installation, plant operators recommended several additional improvements, which required a redesign of the panel.
The redesign-of the panel has been completed, but it has slowed fabrication and installation of the panel.
We are still reasonably assured that this modification will be completed on schedule, although future problems could delay its completion until March 1, 1992.
INDICATOR CRYSTAL REPLACEMENT:
- The resolution of HED's 180 and 331 call for the replacement of the meter face-crystals on various Sigma edgewise meters in the Control Room.
These meters are scheduled to have their nonglare crystals replaced'with regular crystals.
Operators feel that the meter faces are.casier to see t'ith the regular crystals.
This crystal replacement was scheduled for completion by December 31, 1991.
Instead, it will~be completed in-1992, in conjunction with the Point Beach Nuclear Plant calibration program for Regulatory Guide 1.97 meters._ This program is discussed in a letter dated July 5, 1991 (NRC-91-064), and is due to begin during the Unit 1 refueling outage in the Spring of 1992.
METER FACES FOR TURBINE AND ROD CONTROL:
The resolution of HED 31 calls for lengthening th, graduation scale markings on meter f aces associated with turbine go /ernor valve position, control rod bank position, and condenser vacuum.
It has been determined that the meters associated with the turbine and control rod bank position are small meters with relatively small i

Because of the small size, there is no room on the neter faces to make the required modification.
For this reason, the modification will not be performed on these meters.
Additionally, the condenser vacuum meters require the scale graduations to be lengthened to make them easier to read.-
- However, it has been determined that the graduations are not significantly shorter than the required length and the operators have no problems reading them.
Therefore, no further action will be taken with these meters.
For further information on this HED, the revised record is attached.
HYDROGEN CONCENTRATION I.NDICATORS:
The discrepancy described in HED 770 concerns the fact that only two of the four hydrogen monitors displayed on panel-2C20 are pressure compensated.
Further investigation has revealed that all four of the monitors are pressure compensated.
Because the description of the discrepancy was in error, no further action will be taken-on this HED.
For further information on this HED, the revised record is attached.
ROD CONTROL COUNTER RESET SWITCHES:
I HED 72 incorrectly states that it is possible for the operators to remove the key from the associated rod control counter reset switch when the key is not in a safe _ position.
These switches are used to rezero the equipment associated with the control rod bank position counters.
A check of the bank position counters associated with Unit 1 was performed during the recent refueling outage, and it was y
determined that.this key could not be removed when the switch was in the " reset" position.
These switches are spring _ return to center switches, and the key can only be inserted and removed from the center position on these switches.
Therefore, there is no reason to replace these switches For further information on this HED, the revised record is attached.
PROCEDURAL CHANGES:

This HED calls for a Caution statement to be added because the cross-tie the operators proposed to use to energize the safeguards bus has a

After further review of this l
HED, it has been determined that it is not the intent of this procedural step to have the operators use the cross-tie referred to l

To prevent any misinterpretation of this step, the procedure will be revised and training will be conducted.
This procedural change will be completed before the end of this year.
For further information, a copy of the revised HED is attached.
METER FACE RANGE ADJUSTMENTS:
The resolution of HEDs 347 and 438 call for the modification of the meter ranges on the meters associated with Containment Spray flowrate and Residual Heat Removal System flowrate.
These HEDs are covered under modification request MR-88-018*D which was scheduled for completion by December 15, 1991.
In order to complete this modification, several of the affected meters require the installation of new sensor capsules.
These capsules have just been received from the supplier.
Once these sensor capsules are installed, the meters will have to be calibrated.
We are still reasonably assured that these modifications can be completed on schedule, however problems with sensor installation and calibration could delay their completion until April 1,
1992.
HEDs 347 and 438 are both classified safety-significant under the Point Beach Nuclear Plant DCRDR scheme.
We had planned on closing out all safety-significant HEDs by the end of 1991, but there is a s
possibility that these two HEDs may not be completed by this time.
The resolution of HEDs 405 and 530 call for the modification of the meter ranges on the meters associated with Pressurizer vapor temperature and Containment Spray flowrate.
These two HEDs are also covered under modification request MR-88-018*D.
We are still reasonably assured that those modifications will be completed on schedule, but for the same reason discussed for HEDs 347 and 438, their completion could be delayed until April 1,
1992.
SPRAY ADDITIVE TANK INLET FLOWMETEBji:
The resolution of HEDs 49, 532, and 611 originally called for the removal of an unidentified scale from all the Spray Additive Tank inlet flowmeters.
These meters each have two scales, one in black with units of gallons per minute, and the other in red without any units.
Further review has determined that the black scale on the meters refers to water flow in gallons per minute and the red scale refers to Sodium Hydroxide flow in gallons per minute.
Because of this, the resolution of these HEDs have been changed to properly identify both scales on these meters.
These meter face modifica-tions will still be completed on schedule.

The resolution of HED 34 calls for the modification of various meter face pointer tips on numerous Control Room panels to ensure that these tips are within 1/16 inch of the scale graduations in order to meet NUREG 0700 guidelines.
'"a possible solutions would be to lengthen the pointer arms of the a"'ected meters or replace all the maters.
It has been decided that lengthening the pointer arms could have an adverse effect on meter reliability, and that replacing the meters is not a cost effective solution.
Since these meters are not-safety significant, it has been decided that no changes will be made because the meter face pointer arms are close enough to.the scale graduations to allow the operator to take readings within the required accuracy requirement $o The resolution of HED 35 requires mod.ifications to be made on all 66 Rod Position Indicator (KPI) pointer arms te prevent them from overlapping the sca2e graduations on the meters, It has been decided not to change the pointer arms because the operators feel that this overlap is necessary to aid in the proper alignment of the control rods.
The resolution of HED 36 calls for changing the meter face pointer arms of several meters located on panels 1C03, 2CO3, 2C04, Col, and CO2.
Currently, these pointor arms are black in color which is in violation of the Control Room color scheme.
Painting these pointer arms red, as required, could have an adverse effect on meter reliability, so it has been decided to make no changes to the meters.
Leaving the pointer arms black still provides sufficient contrast against the white meter face background and still permits rapid operator recognition of pointer position as recommended by NUREG 0700 Section 6.5.2.2C.
The reso1.ution of HED 51 calls for modification of meter face pointer arms to prevent scale numeral concealment on all 66-analog Rod Position Indicators and Sigma 1251 dual scale edgewise meters located on panels C01, 1CO3, 2CO3, 1C04, and 2C04.
To prevent the possibility of affecting meter reliability, the pointer arms will not be modified on any of these meters.
Instead, the dual scale edgewise meters will have the numerals on the side of the meter faces enlarged.
This modification will be completed by the end of 199.. as_previously scheduled.
This change is proposed because this 1
HED is not safety-significant, and the operator will be able to read the larger numerals by slightly repositioning his sight if they are concealed-by the pointer.
This will be possible because the pointer arm is thin in comparison to the larger numerals.
Also, major scale graduations and numerals that are in view can be

The required modification will not be performed on the 66 analog Rod Position Indicators due to the small size of the meters.
Their small size will not permit enlarging the numerals any further.
