FACILITY:
quad-Cities Nuclear Power Station Cordova, Illinois 61242 IDENTIFICATION OF OCCURRENCE:
Identification of a crack in each of the 4 inch reactor recirculation pump discharge valve bypass lines, 1-0203A-4"A and 1-0203B-4"A.
CONDITIONS PRIOR TO OCCURRENCE:
Unit 1 in cold shutdown at approximately 170 F.
DESCRIPTION OF OCCURRENCE:
In accordance with the provisions of AEC R.O. Bulletin 10A, ultrasonic testing of accessible welds in the bypass piping around the recirc. pump discharge valves was performed.
During. the initial visual inspection of the lines at approximately 4:30 AM on January 10th, water was observed on the upper weldolet on the A loop and a dry racidna was nbserved nn the unner weldolet on the B loop. Ultrasonic testing of both lines was conducted between 6:00 AM and 9:00 PM on January 10th. This was followed by radiographic and dye penetrant examinations all of which were completed by 9:00 AM January 13th.

. side of the weld, and an Indication from the 45 shear wave ultrasonic exam-ination on'the B loop F-1 weldolet running from 10:00'to 2:00, inch to 3/4 inch from the weld bead.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
Equipment Failure The cause of the failed pipe and welds has not yet'been determined. Due to the dncertain cause of this and the other. similar pipe *f' allures,.this letter does not constitue the final: resolution of the problem. The reports sub-

mitted based on 'the studies' regarding the cause or causes of these failures will. apply to this abnormal occurrence report.
ANALYSIS OF OCCURRENCE:
Early detection of the cracks prevented violation of the Technical Specifi-cation ' leak rate limit's set forth in section 3.6.D.1.. No radioactivity was l
~ released. to the environs nor was any Fealth hazard to the pubile or plant i
l' personnel posed. Thus, the safety implications of the occurrence are L
minimal.

CORRECTIVE ACTION:
The existing recirc purnp discharge valve bypass piping on both loops A and B will be entirely replaced.
FAILURE DATA:
During a similar inspection on quad-Cities Unit 2 on September 16, 1974, a crack was found on bypass loop B and repaired, A second ultrasonic inspection, conducted on Quad-Cities Unit 2 on Jecember j
23, 1974, revealed two indications on bypass loop A and one additional indi-cation on bypass loop B.
Both Iines are being replaced during the current Unit 2 refueling outage.
i Similar cracks were also found at Dresden and Millstone stations.
