Dear Mr. O'Reilly:

On May 12, 1978, it was reported that two construction workers received apparent exposures to ionizing radiation in excess of' -

the limits of'10CFR20.101 A and B. Accordingly, an extensive investi-gation was initiated in an effort to identify the source and cause of the exposure. This investigation included the interrogation of the individuals, review of their work locations and schedules, and verification of records associated with radiation control are-entry. Containment entry, dosimeter control logs, and other records were reviewed to ensure that these individuals had not entered -

a radiation control area. Source strengths, schedules, and controls regarding radiography performed during the outage were reviewed to determine the potential for this as a source of the apparent _

exposure. A review of the high voltage test equipment used in generator testing was conducted to determine if X-ray emission was possible.

As a result of the investigation, it is-believed that the exposures reported on May 12, 1978, did not occur to the individuals.

Although the exposures apparently occurred to the thermoluminescent dosimeters (TLD) assigned to the individuals, review of their activities and plant records cicarly indicates that no personnel exposure could have occurred.

It is suspected that TLD badge tampering, through no fault of the individuals involved, could have occurred and resulted in the exposure. However, the events associated with this tampering have not been identified.
