Dear Sir As defined by the Technical Specifications for Quad-Cities Nuclear Power Station, Section 4.7C.1b, the following abnormal occurrence is reported as 1-71-3.

On November 9, 1971 at 3:25 p.m., whilo performing an operClonal hydrostatic test on Unit 1, operations discovered that the integrity of the secondary containment boundary was broken. A 30-inch dinneter hatch was opened in the Unit 2 reactor cavity which opened the Unit i reactor building to the Unit 2 building area which was open to atmosphere.

Upon discovery of the open hatch the hydro-3tatic test was terminated.

'Ihe hatch was closed and testing was performed to assure compliance with secondary containment requirements at 5:15 p.m.

During the phase of testing the Unit i reactor was suberitical and did not contain irradiated fuel.

The reactor building is separated between Units 1 and 2 with construction craft work con-tinuing in the Unit 2 side. A craft foreman had the hatch opened for paintfume ventilation without clearing with the Commonwealth Edison shift engineer ~or foreman.

All craft foreman have been notified that under all conditions approval to change any valve, hatch, or vent duct dampers or blocks position must be obtained from the Commonwealth Edison Company shift engineer or foreman, cM, ',
