Dear Mr. O' Leary:
The purpose of this letter is to inform you of the details concerning the failure of Unit 1 Traversing Incore Probe (TIP) rachine nutber five ball valve.
This abnormal occurrence was previously reported to you by telegram and to Directorate of Regulatory Operations, Region III, in person and by telegram on March 22, 1974.
PRO?LEM At;D IWESTIGATION On March 22, 1974, while perfoming a sp(cial test utilizing the Unit 1 TIP system, ball valve 700-733 fer TIP rachine number five failcd in the open position.
l' nit 1 was operating at 740 E'de at the tice.
The ball valves are part of the Prinary Containment Isolation systen; thus Technical Specification 3 7.D.1 was cxeceded. An orderly shut-down was initiated fifteen minutes after the abnormal occurrcnce ac-cording to Technical Specification 3 7.D.3 An alternative to the orderly shutdown was available according to Technical Specification 3 7.D.2.
However, this alternative was not chosen eince it required use of an explosive fired shear valvc and it was decided not to fire this valve.
Two and one-half hours later the ball valve became operable and went to the closed position.
The shutdown was therefore teminated and the unit was returned to the previous operating conditions.
EVALUATIONS ALT) CORRETIVE ACTIO:3 Safety Implications The ball valve was stuck open for a total of two hours and forty-five minutcs.
If a Group II isolation had occurred while the valve was inoperable, it would still have bcen possible to isolate the penetra-tion by actuating the TIP shcar valve (700-736). V J. F. O' Leary, Director March 29, 1974 Determination of Ca':se A special procedure was written to remove the affected ball vahe and cap the open tubing. When the ball valve was disassembled, it con-tained deposits of a dry type lubricant used inside the TIP tubes.
Similar deposits were previously found when the valves on Unit 1 were inspected and cleaned en June 6,1972 and on Unit 2 on May 30,1972.
CORRECTIVE ACTIONS The number five ball valve on Unit 1 was removed, cleaned and re-as'sembled.
It was then reinstalled and functionally tested.
The other four valves will be inspected during the next regularly sche-duled refueling outage and repaired as needed. Since Unit 2 was shutdown for maintenance during this occurrence, all five ball valves on the Unit 2 TIP system were removed, inspected, cleaned, and then functionally tested after reinstallation. All TIP ball valves (700-733) will be inspected once a year to prevent future failures of this na-ture.
CUMUIATIVE EXPERIEI:CE The last failure of a TIP ball valve frem deposits of lubricant at Quad-Cities Station was twenty-one months ago. Other incidents of sticking valves have been noted.
Implcmentation of an annual inspec-tion and cleaning of these valves should greatly reduce the frequency of future failures.
