Dear Mr. O ' Leary:

The purpose of this letter is to provide you with further information regard-ing the above referenced abnormal occurrence report. The original report described the initial findings and plans related to failures of several jet pump components of the Unit I reactor. The new information provided in this i letter has been discussed with Regulatory Operation's personnel as it became i kncw n .

1. Previous Re ortina Error - There was an error in report-ing in the April 25, 1974, report. Item 4 on page 2 under " discrepancies noted", reported that the restrainer gate wedge on jet pump 5 was missing. This should have read, the restrainer gate wedge on jet pump 6 was missing.  ;

The error occurred while transposing the data from the field check sheet to the draft for the report.

2. Restrainer Gates - All restrainer gate assemblies have been removed. New assemblies are being processed by General Electric Company. These new gate assemblics are due to arrive at the Station by May 25. 1974 3 Restrainer Gate Vedoes - Several of the original restrainer gate wedges fit in place too decoly and did not meet the installation tolerance specified. The wedges for the new gate assemblies have been redesigned to be slightly larger in size. The larger wedges should provide assurance that the required tolerance will be met.
4. Restrainer Gate Bolt Keerers - The restrainer gate bolt keepers have also been slightly redesigned. The new design eliminates a shoulder anc lip on the edges of the keeper.

5. Riser Braces - Tha risar brccss en all t:n jot pump inist j risers have been inspected with the underwater television camera. All brace to vessel wall welds were found to be intact.  !
6. Jet Pumo S 8. 6 Inspection - Jet pump 5 was lif ted approximately 2" from its normal position. The seating surfaces at both the inlet and outlet were inspected with the underwater television camera and found to be in normal condition. Jet pump 6 was lifted from its normal position and raised completely out of the water for inspection. Both the inlet and outlet seating surfaces were found to be in normal condition. Slight wear 6

on the order of a few thousandths of an inch was observed on the q belly band in the vicinity of the contact area with the wedge.

7 Missino and Loose Parts - Since the initial report, some of the .

reported missing parts have teen retrieved from the vessel.

Also, additional parts have been found to be missing, and some parts have been retrieved. Table 1 is a complete index of all parts known to be either missing or loose in the vessel along with their disposition to date. Attempts will be made to locate and retrieve the parts that are still missing. These parts will  ;

be sent to the General Electric Company at their Vallecitos i Nuclear Center, as required, to help determine the possible causes or reasons for the failures.

8. Jet Pump 7 Instrument Line - Several preliminary repair plans to fix the severed instrument line of Jet Pump 7 are being pursued by the General Electric Company. Because of the time c.n..tr.a tn a.u.ing th. tenilag necessery in imp!e-ent the repairs, the fix will not be available for the current outage.

it is planned to operate during this next cycle with the instru-ment li'ne as it now is, and to try to repair it during the next -

refueling outage. A safety evaluation regarding continued opera-tion in this manner will be submitted prior to startup of the unit.

Plans related to these problems are discussed at least daily between Edison and General Electric. Final resolution of all the problems is still pending as of this date. AEC Regulatory Operations personnel have been, and will continue to be updated frequently on the status of the repairs to the Jet pumps. Further information will be reported to you on a timely basis as further developments occur or until the necessary repairs are finalized and this report is closed out.
