The purpose of this letter is to inform you of the details concerning the simultancous opening of the Reactor Building to Turbine Building main floor interlock doorr, thus violating sccondary containment integ-rity. PROBLE! Alm INVESTICATION On April 25, 1974, with unit one in the shut down mode for the current refueling outage, and unit two running at a steady 775 MWE, the door interlock from the Reactor Suilding to the Turbine Building main floor failed. This failure allowed tuo doors to be opened simultaneously, thus violating Technical Specifications 3 7.C.l.
DETEEIt!ATION OP CAUSE AfD COPRECTIVE ACTION The cause of the doors opening simultaneously was determined to be an i
inoperative interlock switch. This switch is located in the interlock area and is used as an override of the interlock in cmergency situations only. Apparently the override had been used, but had not been resct, thus, the interlock was inoperable. This condition was repaired immed-iately by resetting the override emergency switch.
CIEUIATIVE EXPERIEI:CE Previously reported violations of secondary containment have not been caused by failure of the dcor interlock emergency override switch.
