REPORT NUMBER: A0-50-254/74-19 REPORT DATE: June 21, 1974 OCCURRENCE DATE: June 13, 1974 FACILITY: Quad-Cities Nuclear Power Station Cordova, Illinois 61242-IDENTIFICATION OF GCCURRENCE:
Condenser Pit five-foot level switch failed to actuate Control Room Annunciator during monthly testing.
CONDITIONS PRIOR TO OCCURRENCE:
1.
Unit I Reactor in cold shutdown.
2 All three Unit I Circulating Water Pumps out of service.
DESCRIPTION OF OCCURRENCE:

of-service cards were placed on the three Circulating Water Pumps to pre-clude operation until the apparent malfunction was corrected. A work request was imediately initiated to investigate and repair the apparent malfunctioning trip and alarm circuitry.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The apparent cause of the failure of the surveillance test of LS-1-4441-25A to actuate the Control Room Annunciator is that the Level Switch Float l
Plunger was not fully lifted vertically during testing, thus not clo::ing l
the switch contacts.
ANALYSIS OF OCCURRENCE:
Investigation inilcates that no switch failure and thus no occurrence actually occurred.. Therefore, this incident has no safety implications.
CORRECTIVE ACTION:
Purther testing and investigation shewed the icyc1 cvitch trip and alam I
circuitry to be fully operable. The out-of-service cards on the Circulating l
Water Pumps because of this occurrence were removed. Operators vill be in-structed in the proper mothod of actuating these level switches for future testing.

FAILURE DATA:
The five-foot Condenser Pit Level Switches have performed satisfactorily in previous tests. A similar incident also occurred on June 13, 1974 in testing Unit 2 five-foot Condenser Pit Level Switches (Reference A.O.-
50-265/74-11).
