Unit 2 Standby Liquid Control System "A" relief valve setpoint drift.
' CONDITIONS PRIOR TO OCCURENCE:
Unit 2 reactor critical at 530 MWE with normal water level and temperature.
DESCRIPTION OF OCCURENCE:
While performing relief valve setpoint tests for unit 2 standby liquid control system, at 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> on June 25, 1974, the "A" relief valve was found to lift at 1520 psig, which exceeds the Technical Specification 4.4.A.Z. limit of 1400( p 4 1490 psig7 The standby liquid control system pressure gage was re-calibrated at 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br /> and the A relief valve was reset to lift at 1460 psig.
Tac D relief valve was chac' rad and roundite relicyc at 1400 psig.
At 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br /> on June 26, 1974, the A relief valve was reset to lift at 1440 psig and D was set to lift at 1430 psig to allow for setpoint drift.
1 DESIGNATION OF APPARENT CAUSE OF OCCURENCE:

r This Abnormal Occurence is attributed to setpoint drift of the "A" relief valve.
., a ANALYSIS OF OCCURENCE:
The impact of the 1520 psig relief valve setpoint drift is minimal on the operation of standby liquid control system.

for continued pumping sodium pentaborate solution into the reactor' in the event that the standby liquid control system would have been required.
CORRECTIVE ACTION:

- 1 Action Item Record No. 4-74-57 was issued on June 13, 1974, to investigate the cause of standby liquid control relief valve setpoint drift and determine _ corrective action.
This investigation
'is presently in progress by Quad Cities Station and the Crosby Valve and Gage Company.

'AILURE DATA:
Standby ligdid control relief valve setpoints hhhe been found to be out of specification limits twice before at Quad Cities Station.
In June of 1973', unit 1 reliefs were found to be set low.
During refueling outage tasts in May of 1974, unit 1 relief valves were found to relieve "higlier than specification limits.
