Sections 1.0.A.2, 3.6.G, 6.6.A.3, and 6.6.B.3 The purpose of this letter is to inform you of the details regarding the jet pump failure at Quad Cities Unit 2.
This incident occurred on August 20, 1972 at 11:00 p.m.
D_ESCRIPTIO_N_ _0_F_ INCI_ DEUT With Unit 2 at Quad Cities Station operating at 60% thermal power, a recirculation loop flow unbalance approaching 15%
was detected.
Further investigation revealed abnormal flow indication on Jet Pump #17 I_MMEDI_AT_E _ A_CTIONS_
Jet Pump #17 was-declared inoperable and an orderly shut-down of Unit 2 reactor was started at.12:15 a.m.
on August 21, 6
1972.
All rods were inserted by 5: 10 a.m.
INVESTIGATION When Unit 2 reactor vessel head was removed, it was dis-covered that Jet Puc:p #17 ha.d been displaced from its K-normal voastion and vs= leaninc on 3et rump #16-3et Pump #17 was removed from Unit 2. reactor on-August 27,
