Dear Dr. Murley:

SIGNIFICANT CONSTRUCTION DEFICIENCY JACKET WATER CHECK VALVES HOPE CREEK GENERATING STATION On March 14, 1984, a verbal report was made to Region I, Office of Inspection and Enforcement representative, Mr. E.

Kelly, advising of a potentially significant construction deficiency concerning jacket water check valves manufactured by GPE Controls that were installed on our Emergency Diesel Generators by Colt Industries. The following final report is provided in accordance with 10CFR50.55(e).

Description of the Deficiency Bechtel Power Corporation, our Architect / Engineer and Constructor, has advised us that a GPE Controls jacket water check valve failed during factory testing of a Diesel Genera-tor set for Public Service Company of Indiana. The noted failure was described as " disassembly in service". Four of the same GPE Controls check valves had been installed on our Hope Creek Diesel Generators. Colt Industries has filed a Part 21 report with Region III regarding this problem.

Safety Analysis The Emergency Diesel Generators are safety related and supply power to engineered safeguard equipment and other safety related equipment required for safe shutdown of the 8405010499 840413 PDR ADOCK 05000354 S PDR ._.

Had this problem gone undetected and uncorrected, there was the potential for loss of one or more of the four standby Diesel Generator i sets. We, therefore, consider this condition to be reportable in accordance with 10CFR50.55(e).

Corrective Action The four jacket water check valves have been removed from the Emergency Diesel Generators and returned to Colt Industries in accordance with their instructions. The valves will be reworked by GPE Controls in accordance with Colt's Part 21 Report.
