EAR GENERATING STATION FORKED RIVER, NEit JERSEY 0873' . Abnormal Occurrenco Report flo. 50-219/74/ 32 IDE!GIFICATION Violation of the Technical Specifications, paragraph _ N/A OF OCCURTINCE: .It was observed that the RV400 core spray booster pu p pressure ' switch failed in the permissive position. This created a condi-tion whereby had core spray booster pu=p NZ038 failed to start or failed to catabli:h a dischargo pressure of 230 psig its associated redundcnt booster purp, NZO3D, would not have started auto:atically. 'this event is censidered to be an abnormal occurrence as de- ~ fined in the Technical Specifications, paraZraph 1.15D CONDITIONS PRIOR TO OCCURRENCE: _ Steady Stato Power Routino Shutdown _ llot Standby Operation Cold Shutdown Load Chaages During X Refueling Shutdown .Routino Pcwor Operation < i.l. • Systen'II coro spray booster pu p initiation logic, was,in the

,.. ~ energized position.. An investigation indicated that the System II core spray booster pump dischargo pressure switch, RV40D, which sctuates.this relay was in the closed position. The switch

w w moeou e.ue wwo m u. ~ uio piu uiu was u N' up t O 2 "st 230 psig, thereby energng the 1K1140 relay. Norcally closed contacts from the 1K114D relay act in conjunction with the IK114B zul,ay and RV40B pressure switch to trip the NZ033 boostor pu p and start the.re'dundant NZO3D booster pu p if the pucp does not co:o up to.230 psig after five seconds. Failtire of the switch in the closed position created a condition whereby NZO3B would not have tripped if it-falled to start or l establish'suffici'ent' dischargo pressure and pu p NZO3D would not '. ' $avo started since the logic was-failed in the " satisfied" position. > y,,'. ' ,t It should be noted 'that this ' failure occurred subsequent to the <J April.' 12 plant shutdown since auxiliary contacts from this relay 3 ', actuato an alarm in 'the station control roon which was not ob-served during poWor operation, hhen tho' plant is in the col'd , shutdown conditien, this alarm is norna11y on because -it is a . cocoon alarm for the 285 psig percissive switch, RE17 for. the core spray para 1101 injection valves. Since the reactor.was fully depressurized,. this switch was. closed and tho akara was on. i. ,', ; e APPAPIh7 Ct.USB Design Procedure OF OCCURRENG: }!anufacture Unusual Servico Condition Installation / Inc. NIA1,YSIS OF The safety significance of this event 7s consF&re @ 9 OCCURRENCE: minimabince the reactor was in.tho shown condition with the 5 c'avity flooded. Had a IDCA occurred in this condition, the Systen II. core spray pu=ps which operated satisfactorily would have delivered core spray flow at rated capacity but at a lower discharco pressure, assuming the booster pu:p failed to start. ~ ~. - - - - ..,.m- ^ au :m..m:?. v.w:ne;vr:rmv. -,v e.,,.n. v. The 16h114D rolay was placed in the deenergi::ed position which 00RRECTIVE ACTIO:l: is the tripped condition. In this configuration, RV40B would .have acted to sense ir. proper booster pu:p discharge pressure m ' dnd started the redundant pu=p. I t..
