205h5 Dear Mr. Kreusi On September 12, 1972 Consolidated Edison notified the Commission by telephone that a Steam Generator nozzle debris plug assembly had been inadvertently left in place during a hydrostatic test of the Reactor Coolant System.
Enclosed herewith is a renort entitled " Investigation and Con-sequences of a Steam Generator Nozzle Debris Plug Assembly Being Inadvertently Left In-Place During Operation of the Reactor Coolant System" dated October 11, 1972 for the above-referenced plant.
