Newberg, Inconsistencies in the Documentation of Structural Steel NRC Docket Nos. 50-456/457
Dear Mr. Keppler:
On March 23, 1984, the Commonwealth Edison Company Project Construction Department notified Mr. Paul Telke of your of fice of a potential deficiency reportable pursuant to 10 CFR 50.55(e).
This deficiency concerns inconsistencies found in certain documentation of the structural steel installed by G.
K.
Newberg at our Braidwood Station.
For your tracking purposes, this deficiency was assigned Number 84-05.
This report fulfills the thirty day reporting requirements of 10 CFR 50.55(e), and is considered to be an interim report.
DESCRIPTION OF POTENTIAL DEFICIENCY An overview of certain Structural Steel Installation Traveler (SSIT) packages has revealed some inconsistencies in the documentation which include:
heat numbers that have been changed on the fabrication tickets, recorded length of welds on weld inspection requests that have been changed, and use of alternate details that were not noted.
These changes have been lined out, initialed, dated, and there is no explanation as to why these changes were made.
ANALYSIS OF SAFETY IMPLICATIONS The inconsistencies found in these traveler packages do not imply that the work was performed incorrectly, that records were altered, or.that the safe operation of the plant would be affected.
However, these inconsistencies do need to be reconciled.
It is our intent to assure that all requisite safety related structural steel installations have a' complete and credible record of installation.

-2 CORRECTIVE ACTION TAKEN AND DATE OF COMPLETION A review of all SSIT packages generated by Gust K. Newberg will begin upon the Commonwealth Edison Company approval of G. K.
Newberg's document review procedure.
The procedure is l
scheduled for submittal to the Commonwealth Edison Company by April 19, 1984 and approval is expected by April 27, 1984.
G. K. Newberg is expected to complete the reconciliation of the inconsistencies within the documents by November 15, 1984, at which time a final report will be issued.
