BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - QA TEST DOCUMENTATION ON RADIATION MONITORING EQUIPMENT - BLRD-50-438/82-62, BLRD-50-439/82 FINAL REPORT The subject deficiency was initially reported to NRC-0IE Inspector F. Cantrell on August 24, 1982 in accordance with 10 CFR 50.55(e) as Audit 82V-29, Deficiencies 3 and 4.

This was followed by our interim reports dated September 22, and November 9,1982, and January 10, and May 11, 1983 Enclosed la our final report.

TVA does not now consider the subject nonconforming condition adverse to the safe operation of the plant. Therefore, we will amend our records to delete the subject nonconformance as a 10 CFR 50.55(e) item.

FINAL REPORT Description of Deficiencies During the TVA audit of GA Technologies, Inc. (formerly General Atomic Company), San Diego, California, the following deficiencies were noted in the implementation of their previously accepted 'QA program for TVA contracts:

Deficiency 3 Contrary to section 11, paragraph 5.3, of GA Technologies' QA manual, a test operator did not completely record the test results for a radiation monitor (TVA item 03513501-001) already shipped to TVA for use at Bellefonte.

Deficiency 4 Contrary to ANSI N45.2, section 18, which requires results of inspections to be identifiable and retrievable from records storage, QA records from GA Technologies' Cimex facility do not provide for the required identification and retrievability of inspection results.

GA Technologies has also supplied radiation monitoring equipment for other TVA nuclear plants.

Safety Implications TVA's Office of Quality Assurance has determined that the incomplete test document identified in deficiency 3 is an isolated occurrence No involving only the one low level monitor identified above.

corrective action beyond the standard operability testing which will be performed by the vendor is required for this item.

Deficiency 4 identified a lack of retrievability for inspection documentation of subassemblies received by GA's California facility from their plant in Mexico. Because the completed units are tested in San Diego, and GA's test program has been determined to be adequate, there is no indication that faulty equipment has been used in any TVA plant.

Because neither deficiency represents a significant breakdown in plant quality assurance and no significant deficiency in design or construction or significant deviation from performance specifications exists, TVA now believes that 10 CFR 50.55(e) does not apply to these items.
