Description of Deficiency Vendor audit 83V-66 was conducted at Atwood and Morrill Company, Salem, Massachusetts, during the period of August 8-11, 1983 As a result of this audit, deviation 83V-66-2 was initiated which identified the lack of adequate quality assurance requirements imposed on purchase orders for the manufacture of class 1E solenoid valves and air-operated valve actuators. TVA contract No.
82K21-830319, Appendix B, " Quality Assurance Requirements for ASME Section III Valves, requires ANSI N45.2, " Quality Assurance Program Requirements," to be imposed on the valve actuator manufacturer.
