O ENCLOSURE BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 SEISMIC PIPE SUPPORT INSPECTION DEFICIENCIES NCR 2208 10 CFR 50.55(e)

BLRD-50-438/83-18, BLRD-50-439/83-14 FINAL REP 0hT Description of Deficiency Inspections of rigorously analyzed pipe supports were not correctly performed. The support installation locations were not verified against the seismic analysis isometric locations for those cases when the installed location deviated from TVA's Division of Engineering Design's design detail locations. The apparent cause of this deficiency was a misunderstanding of paragraph 7.1.b of BNP-QCP 6.17, which requires the verification of the location deviation.

Safety Implications The installed location of certain pipe supports may be further away from the presumed analysis location than the EN DES relocation tolerance allows. These mislocated supports could possibly invalidate the seismic analysis of safety-related systems, to a degree proportional to the number of supports located outside of the design allowables. This situation could produce a condition adverse to the safety of operation of the plant.

Corrective Action Approximately 3800 completed inspection records for rigorously analyzed supports have been identified. These records were reviewed to ensure that the location recorded on the inspection record is within the location specified by the governing analysis document plus any applicable tolerance. The review of these records' has tvsulted in 290 Quality Control Investigation heports (QCIRs) documenting deviations from specified support locations. Approximately 227 of these have been closed and the balance will be closed by October 17, 1984. At this time, all corrective actions will be completed.

BNP-QCP-6.17 has been revised specifying that the support location shall be as shown on the support detail drawings, and any deviation /use of tolerance must be verified against the support load table or isometric. All responsible inspection personnel have been instructed that location tolerances may be applied to the analysis location but not necessarily to the detail drawing location.
