Dear Mr. Gagliardo:

This letter provides an interim report on a 10CFR50.55(e) matter concerning improper fittings found on the low prescure switch of the nitrogen accumulator for the main steam isolation valves (MSIV) at Wolf Creek Generating Station.

This concern was initially reported by Messrs. Maynard and Creel of Kansas Gas and Electric Company to Mr. William Johnson of the Nuclear Regulatory Commission (Region IV) on October 25, 1983.

During the process of removing teflon tape from certain fittings in the plant, the I&C technician doing the work discovered that a fitting on the low pressure switch of the nitrogen accumulator was not the proper fitting. The ferale fitting on the switch, which was supplied as part of the valve by the vendor (Anchor-Darling), was an AMINCO 1/4" O.D. tube connector with parallel threads where the pressure setal is not made on the threads. The male adapting fitting, which was also supplied by the vendor, was a 1/4" NPT pipe thread which utilizes tapered threads where the pressure seal is made on the threads. The combination of these two fittings for the single connection may have resulted in an inadequate pressure seal. Failure of this connection to hold pressure would result in a loss of pressure in the accumulator.

If this condition occurred, the operator would receive a low pressure alarm indicating loss of nitrogen or hydraulic fluid pressure.

An inspection of the other three main steam isolation valves and the four main feedwater isolation valves revealed that the same conditions exist in these valves. All of these fittings will be replaced with proper fittings.
