. VOGTLE ELECTRIC GENERATING PLANT- UNIT 2 TECIINICAL SPECIFICATION SPECIAL REPORT 2-95-1 INVALID DIESEL GENERATOR (DG) FAILURES A. REQUIREMENT FOR REPORT This report is required in accordance with the Vogtle Electric Generating Plant Technical Specifications (TS), section 4.8.1.1.3, which requires all DG failures, valid or invalid, be reported to the Commission in a special report pursuant to TS 6.8.2.

B. DESCRIPTION OF INVALID FAILURES OF DIESEL GENERATORS 2A & 2H During the 2R4 refueling outage, a number of design changes were incorporated on the 2A and 2B DGs. Ten (10) of the eleven (11) failures occurred on DG2B, where the changes were first installed. One modification included installation of a new mechanical speed governor which required a number of starts to adjust and achieve the proper setting. In separate design changes, additional automatic voltage regulators were installed on the DGs and new power source circuit breakers were added to the 4 kV safety busses which exhibited installation and testing problems. The failure of DG2A was the result of a failed temperature sensor. Individual failures for the DGs are outlined on the attached table.

SUMMARY

The DG2B failures have been determined to be invalid because they involved components that had not yet been fully tested and placed into service. A brief description of the cause of each failure is listed in the attached table.

The failure of DG2A was also determined to be invalid since the failed temperature ,

sensor does not trip the engine in the emergency mode of operation.

Following testing, both DGs were declared operable and returned to service.

Diesel Generator 2 A has had 0 valid failures in both the last 20 valid tests and in the last 100 valid tests. Diesel Generator 2B has had 0 valid failures in the last 20 valid tests and 1 valid failure in the last 100 valid tests. Test frequency for both diesel generators remains at once per 31 days in accordance with the requirements of TS table 4.8-1.
