M. Blatt Telephone No.: (914) 526-5127 Unit 2 was at 100% reactor power at the beginning of the report period. On October 4,

Unit 2 began shutdown procedures to inspect the moisture separator reheaters and make appropriate repairs due to their reduced performance. By October 5th the Unit was at a hot shutdown condition.

On October 12th Unit 2 was brought to a cold shutdown condition to repair steam generator primary manway leaks and to inspect the controlled leakage seal on No. 23 reactor coolant punp.

During the outage the major jobs included 1) the inspection and repair of the moisture separator reheaters internals, 2) the inspection and repair of the channel head manways on the hot and cold legs of No. 22 steam generator and the hot leg of No. 21 steam generator, 3) the overhaul of the seal package for No. 23 reactor coolant punp and 4) the inspection and repair of the pressurizer manway.

The reactor was made critical on October 25th.

While critical and approaching synchronous speed a turbine and reactor trip occurred.

This trip was initiated in the substation electrical protection circuitry.

On October 27 while the reactor was at 4% power, and the turbine was at 1750 rpn, the governor control malfunctioned, causing the tarbine and the reactor to be tripped by the independent electrical overspeed protection system. 'Ihe Unit was returned to service at 1419 on the 27th.

On October 28 at 0104 a reactor trip occurred. The Unit was at 60% reactor power when low condenser vacuum required the operator to rapidly decrease electrical output, which in turn caused the Unit to trip on high #23 steam generator level. The Unit was returned to service and for the renainder of the month maintained 100% reactor power.
