Dear Mr. O'Reilly:
I On October 5, 1983 your staff was notified that containment spray system (NS) 2 pump 2B recirculation valve 2NS-8 was found locked open rather than locked closed, and therefore NS train B was considered to have been inoperable since the last time the pump was tested (September 14, 1983). Pursuant to T.S.3.6.2 which is reportable per T.S.6.9.1.10(b), Reportable Occurrence Report R0-370/83-53 was to have been submitted by October 19, 1983. At that time evaluation of the incident and preparation of the report were not complete,and a request for delay of sub-mittal of the report until October 26, 1983 was made by my letter of October 19, 1983. At this time our evaluation is still incomplete. Therefore, the final report will now be submitted by November 4, 1983.
