BELLSFONTE NUCLEAR PLANT UNITS 1 AND 2 - FRONT COVERS SEPARATED FROM BAILEY METER MODULES - BLRD-50-438/83-52, BLRD-50-439/83 FIRST INTERIM REPORT The subject deficiency was initially reported to NRC-0IE Inspector Linda Watson on October 4,1983 in accordance with 10 CFR 50.55(e) as NCR BLN 2454. Enclosed is our first interim report. We expect to submit our next report by February 6,1984. We consider 10 CFR Part 21 applicable to this deficiency.

FIRST INTERIM REPORT Description of Deficiency While inspecting Bailey Meter Company (Wickliffe, Ohio) modules at BLN, TVA's Division of Construction (CONST) personnel discovered that front cover plates for several class 1E modules in control cabinets for the Non-nuclear Instrumentation (NNI) and Essential Controls Instrumentation (ECI) system had become separated from the frame body of the module. The front cover plate i's attached to the frame body by means of two spot welds at the top and bottom of the front cover plate. These spot welds had been broken and had caused the front plate cover to separate from the module. The cause of the failure -is' not known'at this time.
Interim Progress TVA's recommended corrective action is to return the modules to Bailey Meter Company (subvendor) for rework. Babcock and Wilcox (Lynchburg, Virginia), who is the vendor for this equipment, has been' requested, via TVA letter K-7973 to review the subject NCR and respond to TVA with a root cause, corrective action and action to prevent recurrence.
