REPORT OF EXAMINATIONS, INSPECTIONS AND TESTS

. CONDUCTED DURING THE 1983 FIRST REFUELING OF SALEM UNIT II This is the first sixty-day report in accordance with the special reporting requirements of NRC. IE Circular 76-06 and IE Bulletin 79-17. This report is as follows:

1. Unit / Owner Salem Unit II Public Service Electric and l Gas Company
2. Location: Hancocks Bridge, N.J. 08038 l 3. Outage Duration 1/22/83 to 7/31/83 4.

Purpose:

First Refueling

5. Summary: There were no adverse findings in any of the Examinations, Inspections I and Tests conducted relative to IEC 76-06 and IEB 79-17.
6. Examinations, Tests and Inspections conducted during this outage include:
a. Twelve (12) nuclear class II welds in the Chemical

! and Volume Control and Residual Heat Removal Systems were examined by ultrasonic methods during the outage.

b. Portions of the Containment Spray, Safety Injection, and Chemical Volume Control Systems were flushed and i

continue to be flushed during the monthly pump capacity surveillance testing as per Surveillance Procedure SP (0) 4.0.5 (P).

l I c. The PSE&G Maintenance Department under the direction of the Salem ISI Group, conducted an Inspection of all areas in the South Penetration and Auxiliary Building where in the subject piping systems are installed, l

d. Chloride sampling and testing for cloride content at sample points on stagnant piping is conducted on a quarterly basis. During the past 12 months no non-conformances were identified.

Portions of the CVC Operations Systems that do not contain sample points are flushed monthly by the Operating Department in accordance with SP(0) 4.0.5 (P).

e. Visual Service Pressure Leak Examinations were also conducted of the following systems:
1. Safety Injection
2. Residual Heat Removal
