Dear Mr. O'Reilly:

On July 8, 1983, your office was sent a partial report, Reportable Occurrence Report R0-287/83-07, which described an incident which was considered to be of no significance with respect to its effect on the health and safety of the public. -The supplement to this report providing additional details of this incident was originally to have been submitted to you by August 15, 1983. My letter of August 15, 1983 addressed the delay in the submittal of the supplemental report. The August 15 letter stated that the supplemental report would be sub-mitted to you by September 15, 1983. However, the investigation of this incident is incomplete at this time. It is expected that the investigation and supplement to this report will be completed, reviewed and submitted to you no later than October 30, 1983.
