In response to your letter of April 13, 1976, please find enclosed the completed license application forms and details of the items you mentioned.
These enclosures will add to my letter dated March 23, 1976 with license fee submitted at that time.
I have received the refund for my overpayment.
The enclosed application has expanded upon my original re-quest to also add a Dosimeter Calibrator and to perform leak (wipe) tests on other sealed sources.
