Informing you of the accidental death of Control Room Operator Wayne Jones. At approximately 9:00 p.m. on October 15th, an expansion joint ruptured while Wayne was operating a valve on Unit IA - High Pressure Drain Pump. The pump was shut down and was being isolated for maintenance. Heroic efforts by fellow workers succeeded in recovering Wayne from nm area of the rupture and Cardio Pulmonary Resuscitation efforts by two EMT qualified first aid team members were started immediately. All emergency efforts failed to save Wayne's life.

I know you all join me in heart felt sorrow over the loss of our co-worker as we express our sincere sympathy and support to his family and loved ones.

NONRADI0 LOGICAL INDUSTRIAL FATALITY A 23-year-old control room operator trainee was killed instantly at 9:00 p.m. (EST)

Saturday (October 15) when a fist-sized hole blew open in the wall of a 24-inch, carbon isteel pipe attached to the Unit 1 "A" high pressure feedwater drain pump. The man was

down in a valve pit in the auxiliary building to operate a suction valve'and was hit by
360-degree steam from the rupture.
A shift supervisor sustained second degree burns from his knees down during efforts to
remove the man from the pit. The supervisor was treated and released from a local

. hospital.

'The line in which the failure occurred takes water from the feedwater heater drain tank on the secondary side of the plant to the feedwater heater drain pump. The Unit, which is served by seven feedwater heaters, continued operation at 100 percent power.

iThere was no contamination involved in the incident.

iThe NRC resident inspector was notified of the event and reviewed circumstances associated with the event on Sunday. He will closely monitor the licensee's inquiry

into the cause of the accident.

Unit 2 was also operating at 100 percent power at the time and was not affected by the accident.

'The licensee did not issue a news release but has been responding to media inquiries.

The NRC does not plan to issue a news release.

'The Commonwealth of Virginia has been informed, as has the U.S. Occupational Safety and Health Administration.
