Dear Sirs:
On October 4,1991 at 0446, a significant event not ification was made pursuant to 10 CFR 50 Section 72(b)(2)(1) for Unit 1 and 10 CFR 50 Sect. ton 72(b)(1)(ii)(B) for Unit 2.
The call was conservatively mado due to the potential for a leakage path from containment into the annulus through containment spray check valves.
This leakage had the potential to be out side the limits for tho Offsite Daso Assessment and the Control Room Doso Assessinent. Test.ing of theso valves in ihe "as four.d" condition proved the leakage t1 he wit hin the limits. Thorofore, no Licensee Event Report is required to be submit.ted por the above referenced sections of 10 CFR 50.
On November 4, 1991 a meeting van hold to discuss tho opt ions available to address the testing concerns of these check valves.
During this meet ing, t he determination was mado that those check valves woro containment isolation valves and were rnquired to be tested por Technical Specification 4.0.5.
Since this had not been previously recognized, the vnives had not been tested. Thorofore, this is considered a missed surveillance of Technical Specification 4.0.5.
