-BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - LAMBDA POWER MODULE FAILURES -

BLRD-50-438/81-05, BLRD-50-439/81 SUPPLEMENTAL REPORT The subject deficiency was initially reported to NRC-0IE Inspector R. W. Wright on December 18, 1980 in accordance with 10 CFR 50.55(e) as NCR BLN EEB 8006. This was followed by our first Interim report dated January 19, 1981; our final report dated July 21, 1981; our revised final report dated March 23, 1982; our second revised final report dated June 17, 1982; and our supplemental reports dated September 27, 1982; and April 11 and September 12, 1983 As discussed with NRC-0IE Inspector P. E. Fredrickson on January 12, 1984, the date of January 15, 1984 for completion of corrective actions indicated in our final report on this matter would not be met. TVA has discovered that an ACZ6 capacitor has been substituted for a C6 capacitor in the power supplies. A nonconformance was written to document this discrepancy with the recommendation that the power supplies be reworked by the vendor. The corrective action on this matter will be completed by June 1,1984. We consider 10 CFR Part 21 applicable to this deficiency.
